What to Bring
Please bring the following information when visiting a MDL Patient Service Center:

Current insurance identification card (Medicare, Medicaid, private insurance, or HMO/PPO) |

Photo ID (driver's license, employee ID badge) for urine drug screens |
Minors
Minors must be accompanied by a parent or legal guardian for all specimen collections. If you are bringing a minor to a Patient Service Center and you are not the legal guardian, you must bring a statement signed by a parent or legal guardian authorizing you to give permission for the collection and billing of the tests ordered by the minor's physician.
